BODY & FACE CONTOURING
Accent prime helps eliminate excess fat and loose skin, visibly improving the shape and contours of the face and body using the power and efficiency of two technologies –Ultra sound and Radioi Frequency (RF). The cold ultra sound shear waves selectively destroys fat cells while Unipolar RF accelerates the metabolism for fast lymphatic drainage. The application of RF heat energy following fat destruction induces a volumetric contraction effect, tightening the skin and improving its textures while also building new collagen.
Age-related skin imperfections including wrinkles, fine lines, uneven skin tone and skin laxity can be significantly improved with Accent Prime. The treatments effectively address existing indications while also helps to bloster the skin against future aging.
Enlarged individual fat deposits beneath the skin that push up against the underlying connective tissue create the uneven skinsurface and dimpled appearance of cellulite. Accent Prime employs a combination of RF energy and mechanical massage to increase metabolism of adipose tissue – thus reducing fat cell volume, stimulating lymphatic drainage and visibly smoothing of texture of the skin.

*Insulin resistance is global epidemic
*Available evidence suggests that acne is associated with insulin resistance
*Causality and causal pathways between insulin resistance and acne formation needs to be determine by robust studies
*Available evidence suggests that acnegenesis is mediated through IGF-1
ISOTRETINOIN ON INSULIN RESISTANCE
*Isotretinoin causes hypertriglyceridemia which is casually related to insulin resistance
*Studies dd not find isotretinoin to alter insulin sensitivity significantly
*Hypertriglyceridemia associated insulin resistance primarily occurs when lipid is delivered and accumulates in
*Isotretinoin only prevents clearance of triglyceride rich V from blood into tissues.
Exercise on insulin resistance
*Accumulation daily physical activity is a major determination of insulin sensitivity
*Exercise leads to translocation of GLUT -4 to cell mambranes- non-insulin dependent and this action is not following IRS -1 pathway
*Time spent sedentary, time spent in light-activity, and bouts of moderate or vigorous activity does not impact insulin sensitivity independently of total activity
IGF-1 induces SREBP-1 expression and lipogenesis in SEB-1 sebocytes via activation of the Phosphoinositide 3-kinase (P13-K) akt pathway.
HORMONAL CHANGES IN INSULIN RESISTANCE
*Gonadal and adrenal androgen synthesis
*stimulate 5-alpha reductase
*androgen receptor signaling
*sebocyte proliferation and lipogenesis.
What is insulin resistance?
->Reduced sensitivity of metabolic pathway of insulin action after binding to insulin receptor leading to impaired glucose transport, glycogen synthesis and augmented glucose.
Chronologic evolution of insulin resistance.

Clinical Manifestations of IR
There are many reasons why people choose to have face lift surgery:
- Looseness to the skin and relaxation of muscles resulting in wrinkles and folds on the face
- Visible jowls – no longer having a smooth firm jaw line
- Deep lines in the corner of the mouth, which can often give a “sad” look
- Loose skin and wrinkling to neck area
- A lack of self confidence – some people age prematurely and feel that the face does not match their body
- Substantial weight loss in later life can give the effect of ageing – older skin has less elasticity than younger skin and without the fat, will become saggy and wrinkly
- To look more youthful and attractive
The Cosmetic Surgeon may suggest that you lose any weight before surgery to give you the best possible long lasting result. Your Cosmetic Surgeon will also suggest that you stop smoking – an important part in assisting wound healing. If you colour your hair, it is advisable to do so before surgery as it will be several weeks afterwards before it is again safe to do so. It is advisable to take a pair of sunglasses and scarf with you to hospital – this will help to hide any bruising.
If you are a smoker, or take aspirin or any other medication, your Cosmetic Surgeon will wish to discuss this with you at your consultation. Your Cosmetic Surgeon will advise you when you are to discontinue and/or resume these activities.
It is important that your decision to have a Cosmetic Surgery procedure is considered carefully. There are many points that should be considered before deciding to have any type of Cosmetic Treatment or Surgery.
There are also some possible risks, as with all surgical operations, which your Cosmetic Surgeon will talk through and explain to you at your consultation, answering any questions you may have. This is your opportunity to discuss your intended surgery in order that the Cosmetic Surgeon can ascertain your expectations as to what can realistically be achieved.
Every Cosmetic Surgeon who performs face lift procedures for The Harley Medical Group belongs to a relevant professional organisation, is fully accredited to practice in the UK as a specialist in Plastic Surgery, and has specific training and experience in Aesthetic Plastic Surgery.
